Job Description

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 11 states.  We believe in the dignity of every person, and in the freedom of all people to experience the highest degree of self-determination.

We are seeking a Case Manager to coordinate and monitor the assessment and treatment planning process; the delivery and follow-up of individual supported care; and assure that the appropriate service plans are followed. This role also serves as an advocate and liaison.

Job Responsibilities:

  • Advocates for the needs, preferences, and choices of consumers without bias.
  • Assists in pre-placement, placement, and transition of consumers, and secures a residence in the community that meets the individual’s welfare and safety needs.
  • Intervenes for consumers in emergency situations and when services are not delivered.
  • Develops Individual Plan (IP), and ensures consumer assessments are completed to determine if IP goals and objectives are being met.
  • Gathers the necessary information, develops, and maintains planning documents for the consumer’s enrollment, discharge, and service termination.
  • Develops and submits current documents to the proper authority for consumers on level of care, IP, physical site inspection and bill of rights.
  • Supports and encourages the individual supported to increase his/her ability to problem solve and be as independent as possible.
  • Identifies, develops, and coordinates community resources and support.
  • Maintains all documentation of service delivery and billing, and submits service logs to the Executive Director.
  • Initiates a re-evaluation of the level of care when the participant has experienced a significant improvement in his/her ability to function independently in the community.
  • Coordinates Circle(s) of Support for person centered planning.
  • Communicates and documents individual’s progress to the Circle of Support, including the individual, guardian, family, residential staff and other support members.
  • Cooperates with internal and State inspections and investigations.

Knowledge/Skills Required:

  • Bachelor’s Degree in human services or related field with one year of practical experience working with individuals with developmental disabilities.
  • Understanding and commitment to community based support for persons with disabilities.
  • Excellent interpersonal and communication skills.
  • Working Proficiency of Microsoft Office.
  • Valid Driver’s License.

Working Conditions:

  • Frequent lifting, stretching, and other physical exertion during positioning of consumer’s or equipment.
  • Ability to lift 50 pounds (weight of average manual wheelchair when chair is empty).
  • On call, via cell phone, 24 hours a day, in cooperation with other management staff.
  • Work will take place in the administrative office, community, and in the homes.
