We are seeking someone to take on the role of a Community Support Manager/Licensed Practical Nurse. This position is responsible for delivering care to individuals with disabilities in a community setting, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. Furthermore, this individual will ensure best practices are used to teach our individuals how to acquire the physical, intellectual, emotional, and social skills needed to live as independently as possible.

Job Responsibilities:

  • Manage staff: staff evaluations, monitoring and scheduling required trainings, seminars, and conferences.
  • Oversee the residential budget and work with staff schedules to ensure overtime stays within guidelines.
  • Responsible for scheduling Direct Support Professionals and covering shifts as necessary.
  • Works to maintain the consumer's finances, monitor their health, and report on the medical needs of each individual.
  • Monitors fiscal, medical, personal, and ISP documentation to ensure accuracy.
  • Serves as an active member of each person's Inter-Disciplinary Team (IDT), which includes involvement in Individual Habilitation Plan Development.
  • Submits monthly written reports outlining operational status, progress, and concerns. 
  • Prepares progress reports for consumer's immediate support group.
  • Ensures that staff is documenting daily and current plans are being implemented, to ensure our consumer's overall needs are being met (employment, recreational, emotional).
  • Cooperate with internal and State inspections and investigations.

Knowledge/Skills Required:

  • Bachelor’s Degree in related field required.
  • Minimum 2 years' experience in Home Community Based Service or related field. 
  • Minimum 1 year of experience providing nursing services to individuals with disabilities.
  • Able to accurately calculate medication dosages.
  • Valid Driver’s License.

Working Conditions:

  • Ability to lift 50 pounds (weight of average manual wheelchair when chair is empty)
  • Frequent lifting, stretching, and other physical exertion during positioning of consumers or equipment
  • May perform tasks that involve exposure to various medical conditions, communicable diseases, blood, body fluids, or tissues.
  • On-call 24 hours a day for medical emergencies, in cooperation with other management staff
  • Work will take place in the administrative office, community, and in the homes.
