Job Description

Community Options is seeking an experienced and caring Licensed Practical Nurse. The Licensed Practical Nurse (LPN) delivers care to individuals with disabilities in a community setting, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. Our LPN’s are an integral part of our team, developing relationships with our consumers and those closest to them, to improve the overall quality of their lives.

Job Responsibilities:

  • Provide prescribed treatment and health care procedures as ordered by a physician.
  • Creates and distributes Individual Plan Medical/Health Reports detailing participants’ general health, health care goals, and physician recommendations for each participant.
  • Provides documentation regarding individual’s health status for various support people including family, guardian, staff, employment and other professional staff as necessary.
  • Advises supervisor of all concerns and/or issues related to the provision of nursing services.
  • Monitor the consumer's use of medications through Medication Administration logs.
  • Make referrals for needed medical services and confirming that follow-up care is pursued.
  • Instruct all additional support staff on consumer specific medical needs.
  • Provide quarterly in-service training for support staff and Managers.
  • Monitor weight gain or loss and other health conditions of participants.
  • Ensure MAR's are being followed through regular reviews and unannounced checks.
  • Assists in building a support network between the individual, family members, and the community.
  • Maintain health services supplies inventory and places supply orders when the inventory is low.
  • Submit URIs for medication errors or other medical concerns with assistance from Managers.
  • Cooperates with internal and State inspections and investigations.
  • Performs all duties necessary, in compliance with state approved objectives for Home and Community-Based Services (HCS) and Texas Home Living (TxHmL) Consumer Principles for Evidentiary Certification.
  • Other duties as assigned by Registered Nurse and Executive Director.

Knowledge/Skills Required:

  • Bachelor’s Degree, or
    • Equivalent experience in nursing field
  • Graduated from an accredited Licensed Practical Nursing Program
  • Current LPN License in state of employment
  • Minimum of one year of experience providing nursing services to individuals with disabilities.
  • Valid Driver’s License.
  • Understanding and commitment to community-based support for persons with disabilities.
  • Must have experience with Bolus feedings, G-Tubes, and Suprapubic Catheters
  • Knowledgeable about specific state laws and regulations governing practice and continually updates knowledge base.
  • Able to multi-task and switch priorities throughout the day.
  • Excellent writing skills, and the ability to present facts and recommendations effectively in oral and written form.
  • Able to accurately calculate medication dosages.

Working Conditions:

  • Ability to lift 50 pounds (weight of average manual wheelchair when chair is empty)
  • Frequent lifting, stretching, and other physical exertion during positioning of consumers or equipment
  • May occasionally assist with wheelchair transfer of non-ambulatory participants.
  • May perform tasks that involve exposure to various medical conditions, communicable diseases, blood, body fluids, or tissues.
  • On-call 24 hours a day for medical emergencies, in cooperation with other management staff
  • While performing the duties of the job, employee travels by automobile and is exposed to changing weather conditions.
  • Work will take place in the administrative office, community, and in the homes.
