Job Description

We are actively seeking an experienced Licensed Vocational Nurse (LVN) to deliver care to individuals with disabilities in a community setting, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. The LVN effectively interacts with the patient, significant others, and other health team members while maintaining standards of professional nursing.

Job Responsibilities:

  • Provides treatment and health care procedures ordered by a physician and required by standards of professional practice or law.
  • Participates in quarterly and Annual Individual Plan (IP) meetings for assigned participants.
  • Creates and distributes Individual Service Plan Medical/Health Reports detailing participants’ general health, health care goals, and physician recommendations for each participant.
  • Provides documentation regarding individual’s health status for various support people.
  • Monitors participant's use of medications through Medication Administration logs.
  • Makes referrals for needed medical services and ensures that follow-up care is pursued.
  • Provides pre-service training for all new support staff on consumer specific medical needs.
  • Provides quarterly in-service training for support staff.
  • Accompanies individuals to and from community, social, and recreational activities.
  • Assures that health services supplies are available, and places supply orders when necessary.
  • Advises supervisor of all concerns and/or issues related to the provision of nursing services.
  • Assists in building a support network between the individual, family members and the community.
  • Follows strict OSHA procedures regarding blood borne pathogens.
  • Participates in Circle(s) of Support for personal futures planning.
  • Cooperates with internal and State inspections and investigations.
  • Other duties as assigned by the Registered Nurse and Executive Director.

Knowledge/Skills Required:

  • Bachelor’s Degree or equivalent experience in nursing field.
  • Minimum of one year of experience providing nursing services to individuals with disabilities.
  • Valid State Nursing License.
  • Understanding and commitment to community based support for persons with disabilities.
  • Ability to effectively respond to crisis situations.
  • Ability to calculate figures related to medication dosages.
  • Ability to present facts and recommendations effectively in oral and written form.
  • Ability to establish a comfortable and supportive relationship with a person receiving supports.
  • Working proficiency of Microsoft Office.
  • Valid Driver’s License.

Working Conditions:

  • Ability to lift 50 pounds (weight of average manual wheelchair when chair is empty).
  • May occasionally assist with wheelchair transfer of non-ambulatory participants.
  • Frequent lifting, stretching and other physical exertion during positioning of consumer’s or equipment.
  • May be on call, via cell phone, 24 hours a day for medical emergencies, in cooperation with other management staff.
  • May transport participants in own vehicle.
  • May perform tasks that involve exposure to various medical conditions, communicable diseases, blood, body fluids, or tissues.
  • Work will take place in the administrative office, community, and in the homes.
