Job Description

The Medical Coordinator assures the medical needs of our individuals are met and helps to build a support network between the individuals and community members. 

Job Responsibilities:

  • Supports the Community Support Manager (CSM) and assists with individual’s medical appointments - accompanies individuals to and from appointments.
  • Works closely with the Registered Nurse (RN) and CSM to ensure individual medical needs are being met, follow-up is completed, and that all necessary medical appointments are scheduled.
  • Assists in the development of the monthly medical schedule for the residential program with respect to each consumer’s medical needs and scheduled medical appointments.
  • Maintains/monitors inventory of medications, as assigned (Prescription and OTC).
  • Administers medication(s) and completes appropriate documentation.
  • Builds a support network between the individuals and community members/physicians/medical practices.
  • Accurately completes all in-house documentation: maintains current daily/prn notes, and medical consultation reports.
  • Assists individuals, as necessary with daily living skills and the development of independent community skills.
  • Completes training as assigned with individuals towards achieving (Individual Habilitation Plan) I.H.P goals.

Knowledge/Skills Required:

  • High School Diploma/GED.
  • CNA/LPN experience preferred.
  • Minimum 1 year of experience working with individuals with disabilities.
  • Valid Driver’s License.

Working Conditions:

  • Ability to lift 50 pounds (weight of average manual wheelchair when chair is empty)
  • Frequent lifting, stretching, and other physical exertion during positioning of consumers or equipment
  • May perform tasks that involve exposure to various medical conditions, communicable diseases, blood, body fluids, or tissues.
