Starting Rate: $12.50/hr

We are actively seeking a Senior Community Integration Counselor! This role provides individualized services designed to assist participants in managing and overcoming the difficulties confronted by an individual with a developmental disability living in the community.

Job Responsibilities:

  • Assist in managing Community Integration Professionals’ performance under the direction of the day program manager.
  • Monitors and maintains day budget.
  • Monitors and schedules required staff trainings, seminars, and conferences.
  • Monitors fiscal, medical, personnel, and ISP documentation to ensure accuracy.
  • Monitors and schedules vehicle maintenance.
  • Assists in maintaining and monitoring petty cash.
  • Monitors and reports health and medical needs of individuals.
  • Builds a support network between the individuals and community members.
  • Prepares progress reports monthly and quarterly.
  • Controls overtime and budget hours.
  • Coordinates transportation.
  • Serves as an active member of each person’s Inter-Disciplinary Team (IDT), which includes involvement in Individual Habilitation Plan development.
  • Develops individualized daily routine for each participant.
  • Seeks volunteer jobs for participant.
  • Plans at least one weekly group activity.
  • May perform the duties of Direct Support Professionals or CIC as necessary, in accordance with current staffing needs.

Knowledge/ Skills Required:

  • Bachelor’s Degree in related field, or
    • High School Diploma/GED with 1 year of working experience in related field, in lieu of degree.
  • Knowledge and understanding of community resources and local regulatory agency operations.
  • Effective crisis management skills.
  • Excellent interpersonal and communication skills.
  • Ability to present facts and recommendations effectively in oral and written form.
  • Understanding and commitment to community based supports for people with disabilities.
  • Ability to establish a comfortable and supportive relationship with a person receiving support.
  • Valid Driver’s License.

Working Conditions:

  • Ability to lift a minimum of 50 pounds, and to assist an individual physically.
  • While performing the duties of the job, employee travels by automobile, utilizing own vehicle, and is exposed to changing weather conditions.
  • May occasionally assist with wheelchair transfer of non-ambulatory individuals.
  • Work will take place in community and residential settings.
