Seeking Full-Time, Part-Time, and Per-Diem Senior Home Health Aides in Fombell, PA to assist disabled individuals with their daily living skills while encouraging their independence and building positive relationships in the community. 

General Requirements:

  • Bachelor's Degree in Social Work and 3 years' related experience (or combination of both).
  • Valid/Current Driver’s License.
  • Ability to lift 50 pounds.
  • Ability to communicate (verbally and written) with all levels of employees within the company.
  • Must be able to bend, stoop, lift, push/pull, reach, and stand/walk for periods of time.
  • May transport participants in own vehicle (if applicable)  .

General Duties:

  • Work with individuals who require more intensive support, have a higher level of need, and who require caregivers with an advanced level of knowledge and experience.
  • Assists individuals with daily living skills and the development of independent community skills.
  • Utilizes ISP to assess individual's needs and interest in the community, and to develop goals and objectives.
  • Assists in the development of a daily/weekly schedule for individuals to maximize their time spent in the community.
  • Assists with training of new Community Support Staff and provides documentation to Program Manager.
  • Ensures that all Community Support Staff working on the shift with them are trained on all the plans and protocols for the one-to-one intense individual. 
  • Completes training with intensive-need individuals towards achieving ISP goals.
  • Administers medication(s) and completes appropriate documentation.
  • Accompanies intensive-need individuals to and from appointments and activities. 
  • Completes all in-house documentation: maintains current case notes, and monthly reports of one-to-one intensive individual' progress.
  • Responsible to communicate all necessary information occurring on the shift to the next designated caregiver prior to being relieved. 
  • Provides reports to the Program Manager on the individual's progress and how it relates to their overall goals.
